International Exchange Student Application 2008-2009

 Use this form if you are a Case student applying to participate in an exchange program.

International Visiting Student
Application 2008-2009

 Use this form if you are an international visiting student who wishes to participate in an exchange program at Case.
Off-Campus Study Request (pdf) Use this form to request approval to take a course off-campus. Prior approval is required in order for transfer credit to be granted.
Pass/No Pass Option (online form) Use this online form to submit a P/NP ballot
Petition to Re-enroll After an Academic Separation (MS Word) Use this form to request to return to Case after an academic separation.

Voluntary Leave

 Use this form to request to return to Case after you have taken a voluntary leave of absence.

Request for Voluntary Leave
of Absence

 Use this form to request a voluntary leave of absence from Case. It is used if you are NOT enrolled for classes during the semester in which you are requesting the leave. If you are registered for classes, use the Request for Withdrawal Form.
Request for Withdrawal Use this from to request a withdrawal from all of the classes for which are you registered in a semester. If you are not registered for classes and will not be returning to the university (for a semester or longer) use the Request for Voluntary Leave of Absence Form
Special Request or Petition form Use this form to request a special exception to a regulation or curricular requirement.

Transfer Credit Approval Form

 Use this form to have transfer credit approved by an academic department.
Visiting Student Application
Fall or Spring Semester		 Use this form to apply to be a transient/visiting student at Case during the Fall or Spring semester.
Visiting Student Summer Application Use this form to apply to be a transient/visiting student at Case during the summer. This is a "fill-in" pdf form that can either be emailed or mailed back to the Office of Undergraduate Studies.
