Goods and Services
The following must be completed and returned to Case for review in order for your firm to be considered:
1) Supplier Information Form
2) IRS W-9
3) If applicable, please submit certification of your company's MBE or WBE status by one of the following methods: (1) provide a city or state issued certificate or (2) a correspondence from your attorney or accountant.
4) Return all information to:

In addition to any required contract our General Purchase Order Terms and Conditions will apply.
Architect, Engineer, or other Design Professional/Consultant
The following must be completed and returned to Case for review in order for your firm to be considered for work:
1) Supplier Information Form
2) IRS W-9
3) Submit one (1) company brochure noting qualifications of services and capabilities in the field(s) of; historical renovations, schools/colleges, laboratories and/or other specialties as appropriate.
4) Submit one (1)
AIA B431 Architect's Qualification Statement.
5) Submit one (1) copy of your Certificate of General Liability and Professional Liability Insurance on the Acord 25-S form. (Click here to review sample minimum requirements on the Acord 25-S form.)
6) Submit one (1) copy of your Workers' Compensation Certificate
7) If applicable, please submit certification of your company's MBE or WBE status by one of the following methods: (1) provide a city or state issued certificate or (2) a correspondence from your attorney or accountant.
