University Health Service (UHS) is served by several types of professionals, all of whom share an interest in college health. These include physicians, nurse practitioners, psychologists, psychiatrists, and registered nurses. A number of the physicians are affiliated with University Hospitals of Cleveland and with Case Western Reserve University School of Medicine.
All enrolled students are eligible to make use of the services offered within UHS.
Most routine care is delivered by the staff of the General Medical Clinic, and students are seen on an appointment basis except in cases of emergency. When more specialized care is required, students may be referred during fall and spring semester to subspecialty clinics within UHS, such as dermatology, gastroenterology, gynecology, and allergy. For consultation required beyond this, UHS usually refers to physicians and services available at University Hospitals of Cleveland, which is just down the street from UHS.
Care for routine gynecological problems and information about all methods of birth control is available. The staff fits diaphragms and prescribes birth control pills. Nonjudgmental counseling and referral for pregnancy termination is available, as is maternity care referral. Information and treatment for sexually transmitted diseases are provided in this clinic and the General Medical Clinic as well. Counseling for concerns about sexuality/sexual relationships is also available.
A nurse, physician, and member of the counseling staff are available 24 hours a day, 7 days a week, by calling 368-2450. The service assists students in making decisions about illnesses, injuries, and the use of emergency services. Most emergencies are referred to University Hospitals of Cleveland Emergency Room for further care. In case of obvious severe illness or injury, students should go directly to the Emergency Room and then notify UHS. Students will be billed directly for the services they receive. It is their responsibility to initiate insurance claims for these expenses. Assistance in filing claims for those enrolled in the Student Medical Plan is available at UHS.
UHS has a strong commitment to prevention as part of health care and believes that education is the best way to prevent many problems. UHS provides programs on various health-related topics for groups, including talks on the acquired immunodeficiency syndrome (AIDS), date/acquaintance rape, sexually transmitted disease, and cholesterol screening.
"Self-Care Cold," a computer based model for self-evaluation and treatment of colds, is available for use within the UHS. This offers an opportunity for students to make their own decisions about treatment within medical guidelines built into the program. Other services include self blood pressure monitoring, weight check, and visual screening. Computer programs for AIDS education, reproductive health choices, and alcohol information are also available for use within UHS.
Students may continue to receive allergy injections prescribed by their allergist while in school. The extract is supplied by the student and kept at UHS for injection at scheduled appointment times. Students wishing to use this service should contact UHS for more detailed information.
Whenever indicated, the University Health Service staff will notify a student's immediate family in case of serious injury or illness when prior permission has been obtained from the student. The appropriate dean's office will also be notified of any hospitalization. Specific medical information about student illness is confidential and privileged, and will be released only after consent has been obtained, except in cases of life threatening emergencies.
A student treated elsewhere than at the University Health Service should provide a written report from the physician if he or she wishes the University to be notified of the treatment. Such notification is not, in itself, an excuse or waiver of academic requirements, since these are prerogatives of the dean of the school in question.
UHS does not issue class excuses. When a student must be absent from a class, a laboratory, or an examination for medical reasons, it is the student's responsibility to discuss this directly with the faculty member involved. Under certain circumstances, UHS will give students written verification of the date of their visit to the UHS. In the case of students who are hospitalized or who have a prolonged illness such as mononucleosis, UHS will communicate with the appropriate dean's office regarding the illness, but only with the permission of the student.
A medical and immunization history is required before enrollment for all new students. The medical and immunization history becomes a permanent part of the student's medical file.
In order to assure that all student are protected against communicable illness, the following requirements must be met prior to registration:
- Complete Record of Medical History Form. This form is sent to all new students by the offices of their respective deans. It is the responsibility of each student to complete the form and submit it to the Health Service.
- The immunization history must be completed and signed by a physician, or students may send a copy of their school immunization record. Proof of immunity to measles (rubeola) for students born after January 1,1957, and to German measles (rubella) are required for registration at the University. Vaccines will be provided, if needed, for a fee. Registration will be jeopardized if a student has not fulfilled immunization requirements. In some cases, students entering one of the professional schools may have additional immunization requirements.
- A tuberculin skin test is required of all international students, and students in the schools of medicine, nursing, dentistry, applied social sciences, and podiatry. It will be provided after arrival on campus by the University Health Service, unless documented by a physician within six months prior to matriculation.
Students who have not been immunized because of religious belief must provide documentation of church membership at the time of matriculation. They may be excluded from classes and residence halls in the event of an outbreak of such diseases.
Only the student and the professional staff of the University Health Service have access to that student's medical records. Medical records can be released only with the student's written authorization, and then only to physicians, health centers, insurance companies, or whomever the student designates. The records of the Mental Health Clinic are separate from the records of the general health service. Records are kept for seven years after the last entry and then are destroyed.
The Student Medical Plan provides coverage, within the benefit guidelines, for medical services rendered outside the University Health Service. The Student Medical Plan fee is automatically billed each semester to all students. Students who have comparable insurance coverage may waive the medical plan fee. Waivers can be obtained in the Student Accounts Receivable Office or at the University Health Service. Waivers must be submitted each semester and received by Student Accounts Receivable prior to the deadline stated for that semester. Waivers will not be processed after the stated deadline date.
When enrolled in the Medical Plan for spring semester, coverage applies through the summer to the beginning of fall semester.
Students who leave the University, who are covered under the Medical Plan, due to a personal medical condition may extend coverage for one semester (contact UHS for further information).
Students who graduate or leave the University who are covered under the Medical Plan will continue to be covered for medical expenses of a covered sickness or accident for 52 weeks from the date of injury or date of first medical treatment for a sickness, subject to the payment limits of the plan- provided that the accident or sickness was receiving treatment at the time coverage terminated.
